Home Visit & Case Management Services

"Early-On" Program

A collaboration between area intermediate schools, this program is for Wayne County families with a child age birth to 3 years old with developmental delays. Services include evaluation and assessment and a series of personalized home visits focusing on the child's development. Supports to parents are also provided.

"Parents as Teachers"

The Parents as Teachers model provides services to families with children from prenatal through kindergarten. The goals of these at-home visits are to:

  1. Increase parent knowledge of early childhood development and improve parent practices;

  2. Provide early detection of developmental delays and health issues;

  3. Prevent child abuse and neglect; and,

  4. Increase children’s school readiness and success.
